This overview combined nine systematic reviews and meta-analyses, covering 55 trials and nearly 3,750 patients, to assess how effective and safe osteopathic manipulative treatment is across a range of conditions, alongside the quality of the underlying reviews themselves.

What was studied?

The review compared osteopathic manipulative treatment against control treatments for musculoskeletal conditions such as back and neck pain, alongside non-musculoskeletal uses including paediatric conditions, headache disorders and irritable bowel syndrome.

What did they find?

For musculoskeletal pain conditions, osteopathic treatment outperformed comparison treatments for reducing pain and improving function. Evidence was much weaker for paediatric conditions, headache and irritable bowel syndrome. Most reviews reported no adverse events, but review quality overall was rated low.

Why it matters

Osteopathic manipulation's strongest evidence lies in musculoskeletal pain conditions like back and neck pain, the same area where chiropractic evidence is also strongest, and is far weaker for non-musculoskeletal uses.

Why we included it

It shows a pattern that holds across manual therapy professions generally: the strongest evidence clusters around musculoskeletal pain, not digestive or paediatric conditions, where claims still outrun the evidence.
